Conducting an Assessment - The Inspection Questionnaire

This article will show an assessor/point of contact how to conduct an inspection questionnaire assessment, including answering gap analysis questions, providing descriptions, uploading photos, and providing recommendations.

Conducting an Inspection Questionnaire Assessment

An inspection questionnaire is an assessment of the policies and procedures that an organization has in place. These items are typically intangible, and can not be physically inspected. Examples include: background checks, or emergency drills/exercises.

These can even be sent ahead of time to a building point of contact to answer before the physical inspection is even conducted.

 

  1. From the Assessment Details page for the assessment you will be conducting, select the Start/Continue Assessment button in the top right of the screen. This will take you to the assessment module, that allows assessments to be conducted while either online, or offline.

     

     

  2. Once you have navigated to the assessment module, identify that you are on the questionnaire section for the location in which you are conducting the assessment for. The assessment should open up by default to this page. If you are not able to select, Questionnaire, please ensure you are on the building layer from the dropdown to the right.

     

     

  3. You can now assess each item category for compliance/deficiency. You can select an item category to conduct the assessment for this specific category.

  4. There are 3 main objectives when going through and assessing each item category:

    1. Determining the item status (whether it is present or not present at the location being assessed)

    2. Answering the gap analysis questions pertaining to that item category to determine compliance.

       

       

    3. For these gap analysis questions, the assessor will answer them accordingly (Yes, No, N/A (Not Applicable), N/O (Not Observable)). Then, by clicking the “Add More Info” button, the assessor can include supplemental information, including:

      1. Make comments/add a description for further information about the item

      2. Add an option for consideration (also known as a recommendation) for the identified deficiency

      3. Select pre-made options for consideration (recommendations) that have been added to the library, so you can save time and not have to type out repeat options for consideration

      4. Add photos/files if applicable

  5. The inspection questionnaire is complete once all the item categories have been assessed and have a status next to them. This can be observed by:

    1. A “red X” indicates this item is required and missing at this location

    2. A “red exclamation point” indicates the item is deficient

    3. A “gray exclamation point” if the item is missing and not required

    4. A “blue circle with a check mark” indicates that the item is present and compliant at the location.

       

  6. From here, if you have not already begun conducting it, you can start performing the physical inspection, which is outlined in the conducting an assessment - physical inspection guide.
