Creating a New Assessment

This article will explain how to create a new assessment based off the template and standards that are needing to be assessed at a site.

Written Instructions:

  1. Access the Scenario Details page for the Scenario you’d like to perform an assessment for

    1. In the middle of the screen, you can find the scenario you would like to perform inherent risk assessments for, and select the arrow next to that scenario

       

       

  2. You will be brought to the Scenario Detail Page.

    1. By selecting the “Inherent Risk” Tab within the middle of the screen, you will be brought to the inherent risk landscape for the currently selected scenario. From there, you can select the button under the “Actions” Column

     

  3. A new modal will pop up for your create an assessment for the building you have chosen this action column for. Select “Create Assessment” in the top right of this modal.

     

  4. This will bring up the “Create Scenario Assessment” creation experience. The first screen of the “Create Scenario Assessment” experience is Assessment Details. From here, the user will select:

    1. The location of the assessment

    2. The assessment template that will be used for the assessment

       

       

  5. Once the location of the assessment and assessment template are selected, the next screen will fill out the assessment details. On this page, the assessor will input:

    1. Name of the assessment

    2. Description of the assessment

    3. The start and end dates that are required for the assessment

       

       

  6. The third page is the Subsequent Locations page, where the additional location nodes/levels will be selected for the assessment

    1. If there are levels that an assessment should be conducted, the assessor will need to select those here, so that the questions will populate in the assessment to be assessed.

       

  7. The fourth page is the Import Answers and Required Actions page, where the assessment criteria for completion will be selected.

    1. If this is the first time conducting this assessment for this scenario at this location, you will be required to “Start Fresh,” and fill out all the information that is allotted for the question set that has been created.

       

    2. If you have conducted an assessment this location before, you may have the ability to “Import” answers that will populate the assessment with the latest answer received. The questions that will be imported have been answered before in a previous assessment, regardless of the scenario.

      1. You will have the ability to select what answers are going to need to be reviewed and ratified for verification by the assessor, before this assessment being created is closed. For example:

        1. If you select import, and then select compliant and deficient → all questions that have a compliant or a deficient answer will need to be ratified.

        2. If you select import, and then select ONLY deficient → all questions that have a deficient answer will need to be ratified.

    3. Below are a few use cases for an incremental assessments…

      1. A question set was updated/changed and there are new questions that require answers to get answer coverage to 100%

      2. Assessment in Year 1 is a complete Assessment. Assessment in Year 2 to re-ask Questions about Deficiencies, as well as new Questions that have never had their first answer in Year 1

      3. Assessment in Year 1 Winter to re-ask Questions that were N/O in the summer

  8. The fifth page is the Users page, where the assessors who will be assigned to the assessment are selected.

    1. There is no limit to the amount of users who can be assigned to this assessment, but if a user does need access to an assessment, they will need to be assigned it here.

    2. The assessors assigned to an assessment can be edited at a later time if needed, from the assessment details page.

       

  9. The fifth and last page is the Summary page for everything that was just input for the assessment that will be conducted. If the information entered for this assessment is correct, selecting create assessment will create the assessment, and direct the user to the Assessments Page.

     

  10. From the Assessments Page, you will have a few options to start your assessment from this screen.
